Get extra chances to win by referring friends to our office. For every person you refer you will get an extra entry (ie-family of five would be five extra entries, one for each individual not one for the family).

On February 14th, 2010 we will randomly draw a number (we will use this webpage to take out any potential bias). Whichever patient's number in our practice managment program correlates to the random number given will be the official winner. The winner must be 18 years old or older. If the number drawn is the patient number for someone under the age of 18. We will redraw until it matches with someone over the age of 18.

Extra entries. Since we are doing the drawing by patient number, we will keep a spreadsheet of each person's extra entries. When we do the drawing, those extra entries will be numbered starting after our highest patient number. For example, if we have 750 active patients at the time of the drawing, extra entry number one will be "patient 751."

Must be an active patient of record by the date of the drawing. Cosmetic procedures will not be covered. Outside expenses like specialist fees or lab fees will not be covered.

This is not a complete listing of what is not allowed. Treatment decisions will be based on the above guidelines, but will be left to the sole discrection of the doctor. If you have insurance, we may bill your insurance, but no expenses for yourself will be charged.
