To help you with your busy schedule you can download the New Patient documents and fill them out at your convenience. You can either fill them out to email back, or print them off to bring with you to your first visit. If you email them back, follow these instructions, "Open PDF, fill out form, follow instructions on saving completed form by clicking Submit Form in upper right hand corner."

For your signature, just type your name and initials into the signature box.

Authorization Form for previous dental office to release your records to us. You should only complete this form if we advise you to do so. In most cases we can take our own xrays to determine what work needs to be done. In some cases it is better to see previous records to track the history of a problem. Also, several offices charge fees for records duplication [we don't :-)].
